Developing Evidence-Based Sepsis Time Zero Criteria and Quality Metrics

Leverage detailed EHR data from two large healthcare networks to develop evidence-based, objective, and electronically computable definitions of sepsis “time zero”.

Methods for Profiling Hospital Performance Based on Healthcare-Associated Infections

Develop improved methods for hospital profiling and risk-adjustment for HAIs (including hospital-onset sepsis) using novel statistical approaches and detailed clinical data from a large network of hospitals.

Epicenter V: Harvard Pilgrim Health Care Institute Center for Excellence in HAI Surveillance and Prevention Core Project

Conduct research to guide improved management of sepsis, non-ventilator hospital acquired pneumonia, and prevention of C. difficile infection among hospitalized patients.  3/6 aims focused on sepsis.

Electronic Surveillance for Hospital-Onset Sepsis to Expand Detection of Serious Healthcare-Associated Infections

Characterize the incidence, outcomes, and hospital-level variation of hospital-onset sepsis compared to current reportable HAIs; elucidating the impact of hospital-onset sepsis on long-term outcomes; and develop electronic algorithms to identify the specific infections leading to hospital-onset sepsis.

Development and Application of a National Pediatric Sepsis Event Surveillance Definition

Develop and validate a generalizable EHR-based Pediatric Sepsis Event surveillance definition, apply it to a large set of hospitals to estimate the current national burden of pediatric sepsis and recent trends, and create a detailed toolkit to help hospitals conduct local pediatric sepsis surveillance.

Reliability and Validity Testing for a Risk-Adjusted Community-Onset Adult Sepsis Event Mortality Measure

Generate data on risk-adjustment, reliability testing, and validity testing of an updated Adult Sepsis Event (ASE) definition in 3 large datasets in order to support CDC’s submission of a community-onset ASE mortality metric for consideration as a CMS quality measure.

Antibiotic Utilization Patterns and Impact on Outcomes for Patients with Respiratory Viral Sepsis

Determine the prevalence of sepsis attributable to respiratory viruses, quantify antibiotic use in this population and assess their impact on outcomes, and explore the potential utility of viral testing and procalcitonin assays to facilitate early discontinuation of antibiotics.

Identifying Sepsis Phenotypes Associated with Antibiotic-Resistant Pathogens Using Large Language Models and Machine Learning

Utilize machine learning, large language models, and causal inference methods to create predictive models for MDRO infection and overly broad initial antimicrobial coverage; quantify the impact of overly broad and overly narrow antimicrobials on mortality, length of stay, acute kidney injury, and C. difficile infection; and define specific phenotypes of sepsis for which overly broad antimicrobial coverage can be safely avoided.
